We look forward to coming alongside you in any way that we can! This is the first step in letting us know how we can help you. When appropriate and available, the care we provide may include counseling, recommendation for some form of discipleship, or a referral to an external counselor. After submitting this form, please be watching for follow-up emails from a member of our staff who will direct you through your next steps..

We desire to serve you in a timely manner but please note we are out of the office on Fridays and Saturdays. Please allow 3 business days after your submission for us to respond to you. We will communicate with you through email unless you specifically request otherwise in the "Optional" message box at the end of the form.
